Account Creation Request eForm

On November 15, MNsure launched an electronic version of its Account Request Form called Account Creation Request eForm (ACRe). The form is available to consumers who are unable to create an online MNsure account.

Assisters can use ACRe to submit account creation requests on behalf of their clients. It allows direct entry of consumers’ information into an online form and an upload for identity proofing verification documents. Some of the benefits of ACRe include improved accuracy of data entered, increased efficiency for processing forms and the ability for assisters to track the status of forms they submit. ACRe is also mobile-friendly!

Assister access to ACRe can be found on Assister Central on the /assister-central/index.jsphomepage as well as in the /assister-central/helping-consumers/accounts/index.jspCreating Accounts section of Helping Consumers. Assisters will use the same log-in link and credentials that they use to report consumer online application changes. 

If a consumer fails the online account creation process, they can access ACRe on MNsure.org. If a consumer submits the form themselves, an assister will not be able to track the status of the request.

Consumers can still submit the PDF version of the account creation form by mail to MNsure, and assisters are still able to email forms by secure email to the Assister Resource Center (ARC) and Broker Service Line. However, ACRe is the preferred method when online account creation is not possible. Please do not submit duplicate requests through different methods.
