Lesson 4: Eligibility Review

A manufacturer must inform the applicant of the reason why they were denied eligibility for the continuing need program.

If an applicant still believes they are eligible for the program after receiving notification of denial, they can ask the Minnesota Board of Pharmacy to review their eligibility.

Reviews will only be completed for the continuing need program. The Board of Pharmacy will not review eligibility for other manufacturer assistance programs.

Requesting an Eligibility Review

The applicant must fill out an eligibility review request form located on the Minnesota Board of Pharmacy’s website. A certified navigator can help with this activity.

An applicant can also call the Minnesota Board of Pharmacy’s office at 651-201-2825 to have a copy of the form mailed to them.

The following must be submitted to the Minnesota Board of Pharmacy for an eligibility review:

  • An eligibility review form.
  • Copies of documentation that was submitted to the manufacturer when applying for the continuing need program.
  • Copies of any documents that the manufacturer has sent to the applicant.

If the Board of Pharmacy does not receive all of the documents necessary to conduct a review, the Board will ask the applicant to submit the missing documents.

Determinations

The Minnesota Board of Pharmacy will notify the applicant of their decision within 10 business days. The applicant will receive a decision by mail.

The decision of the Board of Pharmacy is final and cannot be appealed.
