Online Account Request eForm

It is wonderful to see so many assisters helping their consumers create accounts by using MNsure’s new online option, the Account Creation Request eForm (ACRe)

We want to provide additional guidance on a few points:

  • A separate request must be submitted for each individual requesting an account be created. DO NOT submit documents for multiple consumers in the same request. Requests with uploads for multiple consumers will be denied and the assister will need to resubmit the requests correctly. 
  • When uploading a PDF Account Request form for the purpose of providing a signature, assister MUST submit both pages of the form. Without the first page, the request is incomplete and cannot be accepted. The signature page alone does not provide authorization from the consumer. 
    • An alternate option to completing the form would be to follow the /assister-central/shared-resources/joint-policies/electronic-signatures-policy.jspElectronic Signature Policy on Assister Central, and have the consumer provide a handwritten “wet” signature. See the section: “Examples of Acceptable Electronic Signatures”. 
  • Save all documents that will be uploaded into the eForm using the following naming conventions:
    • Account Request Form (if required to provide signature): lastname_firstname_dateofrequest_form
    • Verification Document Option #1: lastname_firstname_dateofrequest_ID1
    • Verification Document Option #2 (if required): lastname_firstname_dateofrequest_ID2
    • Signature: lastname_firstname_dateofrequest_signature
