IRS Form 1095-B for Tax Year 2023

After February 1, 2024, individuals who had Medical Assistance (MA) or MinnesotaCare coverage in 2023 can request Tax Form 1095-B as outlined in the options listed in this message.

The IRS Tax Form 1095-B is an informational form only. Enrollees do not need the form to file a 2022 federal tax return and do not need to attach the form to their tax return.

The following options continue to be available to enrollees who want a copy of their Form 1095-B:

  • Enrollees with eligibility in METS who have an online account can view and print a copy from their online account.
  • Request a copy by:

Find the information, including the link to the online web form, on the DHS public webpage about form 1095-B.

The following information is required when making a request:

  • Requester’s first name and last name
  • Address to mail the form.
  • Telephone number
  • Date of birth
  • Case number
  • Names of family members requesting a Form 1095-B
  • Form 1095-B tax year requested.

Enrollees who have changes to their 2023 Medical Assistance and MinnesotaCare coverage after January 02, 2024, will also have a corrected form available online. The corrected Form 1095-B will be available after February 1, 2024. The agency responsible for the case will be responsible for processing the correction when consumers report an error on their form.
