Navigators assisting consumers with completing the Application for MA Coverage for COVID-19 Testing (DHS-7310) over the phone may obtain a telephonic signature from a consumer. Please note that this telephonic signature process may ONLY be used to sign the DHS-7310.
For all other applications, including the DHS-6696, assisters can obtain an electronic (NOT telephonic) signature from a consumer following policy guidance in this bulletin: DHS Clarifies Use of Electronic Signatures for Minnesota Health Care Programs Eligibility Forms.
