Application for Certain Populations

Use this application for special circumstances where a consumer's eligibility cannot be determined online through MNsure. Download this application as an electronic PDF form (DHS-3876). Only use this application if at least one of the following is true:

All applicants are:

  • age 65 or older
  • requesting help only with Medicare costs
  • children in foster care
  • age 21 years or older with no dependents and have Medicare coverage
  • receiving Supplemental Security Income (SSI)
  • applying for Medical Assistance for Employed Person with Disabilities (MA-EPD)

MinnesotaCare Expansion Information

DHS' MinnesotaCare Expansion presentation and the DHS Bulletin 24-21-10 provide information for assisters about the MinnesotaCare expansion.

Additional Information

  • Individuals will most likely need to use one of MNsure's manual account creation options to obtain a MNsure account.
  • Individuals who do not have an open case can apply via the online application or the DHS 6696 (PDF) paper application.
  • Paper application should be submitted to the county of residence for processing. DHS does not process these applications.
  • If a consumer is on an open case with other family members:
    • Case with MA, call county and request coverage
    • Case with MCRE only, call DHS and request coverage
  • Premium questions should be referred to Health Care Consumer Support at 651-297-3862.

  • Medical providers with questions about billing consumers with this coverage should call MHCP Provider Resource Center at 651-431-2700 or 800-366-5411, open Monday through Friday 8 a.m. to 4:15 p.m. (closed from noon to 12:45 p.m. for lunch).
