- $0 unlimited copay for primary care and mental health visits via telehealth
Access Plans
- Two new, more affordable plan options at bronze and gold levels

Copay plans: Core, Bronze Access (NEW), Bronze, Silver (ENHANCED), Gold Access and Gold
HSA-compatible plans: Bronze and Silver
Choose from two networks in the metro
- If members live in the 10-county metro area, they can choose between UCare’s broad network or our more focused network with M Health Fairview
Near statewide service area
- One of the largest networks of health care providers, so members can keep their doctor
- More than 47,000 primary and specialty care doctors and clinics

Mayo Clinic Health System (MCHS)
- Fully in-network for UCare IFP
- No referrals or PA’s needed
- Includes hospitals in Red Wing, New Prague, and Mankato
Mayo Clinic Rochester
- Limited access for complex specialty care only
- Non-network, unless:
  - Member referred by Mayo provider or
  - Member referred by other provider and has UCare Clinical Services authorization
