These renewal forms are sent to the Department of Human Services (DHS). MNsure is unable to process any changes on cases that are currently in renewal.

Income Limits for APTC: 2026 is the first year we have seen since before Covid, in which a case can be unassisted qualified health plan (UQHP) due to being over income limits. It is possible for a case to have received insurance assistance in 2025, but by 2026, it may transition to UQHP status. This chart serves as a valuable tool for understanding eligibility.
