Navigator Case Association Form Reminders

The ARC has noticed an uptick in a couple of errors when navigators are completing the Navigator Case Association Form. Two errors that are frequently seen:

  • It is quite common that question number 3 on page 1 (“Are you submitting this form because you assisted an inmate with completing a Minnesota Health Care Programs Application”) is answered with “yes” when in fact, the assister is NOT reporting an inmate application. This results in a waste of time as staff researches the case and can delay issuing payments.
  • Question number 6 on page 2 (“Applicant Date of Birth”) is commonly answered with the current (submission) date instead of the consumer’s DOB. Submissions with this error cannot be processed due to mismatching personally identifiable information (PII).
