We are excited to see that so many assisters and consumers are utilizing the new eForm for account requests! After monitoring the use of this form, we want to provide additional guidance on a few points:
Signatures: When uploading a PDF Account Request form for the purpose of providing a signature, assisters MUST submit both pages of the form. The signature page alone does not provide authorization from the consumer.
An alternate option to completing the form would be to follow the /assister-central/shared-resources/joint-policies/electronic-signatures-policy.jspElectronic Signature Policy on Assister Central, and have the consumer provide their authorization using a handwritten “wet” signature.
Identity Verification Documents: MNsure has expanded the list of acceptable identity verification document options and made updates to some as well. Before completing an account request eForm, review the /assister-central/helping-consumers/identity-proofing-and-verifications/index.jspVerifications Section in Helping Consumers on Assister Central.
Note: Identification documents issued by a non-US government entity may result in a slight delay in processing your request form as MNsure may need extra review time to process them.
Account Request Form (PDF): If a consumer is unable to create an online account or to use the account request eForm, an /assets/AccountRequestForm_tcm34-182083.pdfAccount Request Form (PDF) along with required documentation can be submitted to MNsure. This form has been updated with the new identity verification options and is also available in /forms/account-request-form.jspHmong, Russian, Somali, Spanish and Vietnamese.
