Since the new Account Creation Request eForm has proven to be a huge time-saving success for assisters and MNsure alike, we will be discontinuing the use of fax and email as options to submit Account Creation (PDF) forms as of February 1. If assisters are unable to use the online form, the PDF form will be accepted via USPS mail.
