Effective June 1, 2024, the Minnesota Department of Human Services (DHS) Health Care Consumer Support (HCCS) will no longer accept any documents from Hospital Presumptive Eligibility (HPE) and Minnesota Family Planning Program (MFPP) providers by fax. 

HPE and MFPP providers must use the HCCS Document Portal for Partners and Providers to submit documents to DHS. This includes:

• HPE forms, applications, and supporting documents.
• MFPP presumptive eligibility forms, applications, renewals and supporting documents.
