It is now available in /assets/account-request-form-hmong_tcm34-432235.pdfHmong, /assets/account-request-form-russian_tcm34-432236.pdfRussian, /assets/account-request-form-somali_tcm34-432283.pdfSomali, /assets/account-request-form-spanish_tcm34-432292.pdfSpanish and /assets/account-request-form-vietnamese_tcm34-432574.pdfVietnamese. These translated versions, as well as instructions on how to assist consumers through the account request process, can be found in Helping Consumers on Assister Central in the /assister-central/helping-consumers/accounts/index.jspAccount Request section.

As a reminder, the updated version of the Account Request form has been condensed to a one-page fillable document, featuring the signature and date line on the first page. Additional informational pages are included, but they do NOT need to be submitted to MNsure with an account request. The second and third pages specify all the accepted options for verification documents.
