Account Request eForm Tips

Registration, logging in and using MNsure eForms:

  • Assisters will use the same account credentials for account requests that they use for reporting application changes. 
  • After logging in, it may be necessary to click on “Manage My Account” to get to the eForms homepage. After landing on the homepage, assisters will be able to switch from the reporting changes form to the account request form.

Signature requirements:

When a consumer works with a MNsure-certified assister, the /assister-central/shared-resources/joint-policies/electronic-signatures-policy.jspElectronic Signature Policy must be followed when submitting their signature authorization for account creation to MNsure. 

Acceptable uploads:

  • Image of a handwritten signature accompanied by a written statement can be uploaded electronically into the eForm. The statement must verify that the consumer’s intent to sign is clear and include a signature and the date signed. 
    • For example: "I, [insert consumer’s name] understand that I am signing the MNsure Account Request form and providing authorization to create an online MNsure account." 
  • Electronic signature captured by a software product that complies with the requirements of the Electronic Signatures in Global and National Commerce Act (ESIGN), submitted with a completion certificate, audit record or similar audit trial document.
  • A PDF version of Account Request form can be uploaded to provide a signature authorization. It is required that both pages of the form to be uploaded. Without the first page of the form the request is not complete and does not provide complete authorization.

Checking status:

When logging into their account, all account creation request that an assister has submitted through the eForm will display under the results section. 

The following are the steps with explanations that an account request will follow in the process to completion:

  • New: Requests that have been entered into the eForm but have not yet been reviewed by a member of MNsure staff. 
  • Able to Process: Requests where identity verification documents have been reviewed and are acceptable. These requests are ready for processing. 
  • Processing: Requests that have been sent to MNIT for account creation. 
  • Complete: Requests that have been processed by MNIT and the consumer account has been created. Notification is sent by email if one was provided or mailed to the consumer’s listed address.
