Recognizing that advancing health equity is a collective effort, the Minnesota Department of Human Services (DHS) will bring together a diverse group of partners including community-based organizations, faith-based organizations, navigators, managed care organizations, health care providers, Tribal Nations and people, Minnesota counties and DHS staff to reflect on our collective work centering health equity and plan for sustaining our statewide collective health equity efforts moving forward. We hope to facilitate knowledge-sharing, listening, collaboration and consensus building, and actionable commitment for sustaining statewide collective health equity efforts moving forward. As a MNsure-certified navigator/certified application counselor, you bring an important perspective based on your connection to community and your professional and lived experience. Please join us to share your perspective, connect with other partners engaged in advancing health equity, and help inform health equity priorities and action items.
