Who Gets this Notice

Consumers whose applications were closed because the changes they reported could not be processed.

What You Need to Do

  • Reapply with your household’s updated information as soon as possible. (Do not create a new account.) This will give you an updated eligibility with the financial assistance you are eligible to receive.
  • Do not delay! If you do not reapply to get an updated eligibility, your enrollment could be delayed, or existing coverage may be terminated.

How to Identify this Notice

On the first page of this notice you will see:

  • The MNsure logo.
  • The title is "Health Care Notice."
  • "MNsure Form C11" on the lower left.


If you have questions about this notice, call the MNsure Contact Center at 651-539-2099 (855-366-7873 outside the Twin Cities) and reference "MNsure Form C11."
