• Include your first and last name and case number on all documents you send.

How to Identify this Notice

On the first page of this notice you will see:

• The MNsure logo.
• Title is, "ACTION REQUESTED: PROVIDE DOCUMENTATION."
• "MNsure V20" on the lower left.
