Who Gets this Notice

Consumers applying for a special enrollment period who need to send verification (proof) of their qualifying life event.

What You Need to Do

  • Send MNsure the documentation needed to verify your qualifying life event as soon as possible: You will not be able to select a plan or enroll until MNsure verifies your qualifying life event.
  • If we do not receive your documentation by the due date, you may miss the chance to enroll in or change plans under this special enrollment period.

How to Identify this Notice

On the first page you will see:

  • The MNsure logo.
  • The headline is, "Action Required: Documentation Needed for Reported Qualifying Life Event."
  • "Enrollment Notice # EE061” on the lower left.
