Who Gets this Notice

Consumers who qualify for a special enrollment period (SEP) are sent this notice to tell them their SEP is now open.

What You Need to Do

Enroll in or change health insurance plans before your SEP window expires. The expiration date is on page one.

How to Identify this Notice

On the first page of this notice you will see:

• The MNsure logo.
• The headline is, "Act Now: Special Enrollment Period Open."
• "Enrollment Notice # EE020” is on the lower left.
