Gather What You Will Need
Having this information ready will help you finish your application in one sitting.
For Everyone in Household
- Birth Dates: For you and anyone in your household, even if they’re not applying
For Each Person Applying
- Social Security Numbers: For anyone who has one
- Driver’s license, Tribal ID or other identification
- Employer Info: Employer's name, address and phone number.
- Income Info: Recent pay stubs, W-2 form and your most recent tax return. Documents for other sources of income like social security or unemployment.
- Current Health Coverage: Information about any insurance you have now. Learn more about what plans count as minimum essential coverage
- Job-based Insurance: You’ll need to report any employer-sponsored health insurance, even if the person applying is not enrolled in it. Ask your employer for help filling out the Appendix A: Health Coverage from Jobs form (PDF).
For Immigration Status
- ID Numbers: If you are not a U.S. citizen, have your SEVIS ID or alien number (A-number) ready.
- Immigration Papers: Have your Green Card, work permit, or other visa handy.
